Self Referral

You can now refer yourself directly for Physiotherapy Assessment at your local Health Centre.

This means that if you are 16 years old or over and live in Scotland you do not need to see your GP first, simply pick up a self-referral form at reception or download it below, fill it in and return to reception. The Physiotherapy department will be in touch when they have an appointment for you.

Please note, this does not mean you will necessarily be seen quicker, and you do not need to attend a GP appointment. Please read the Information Leaflet for more detail.

If you work for a company in the Borders which has less than 250 employees or if you are Self Employed then you may be able to have rapid access to Physiotherapy, Psychological Services and Occupational Therapy - Tel 01896 825983 or pick up a leaflet.

NHS Borders Employees can receive treatment via the Occupational Health Physiotherapy Service - Tel. 01896 825982
